We support seniors leaving hospital, building collaborations between the healthcare system and community resources. Our goal is to support healthy aging in the community and enhance the quality of life for adults aged 55+.

Clients are referred from PCNs, Alberta Health Service (AHS), family physicians, pharmacies, and other medical professionals. Our outreach workers meet with clients in their homes, together we coordinate a plan to support independent living in the community.

Aging in Community Supports (AICS) is a collaboration of four agencies (JFSC, Calgary Seniors’ Resource Society, carya, and Calgary Chinese Elderly Citizen’s Association).

We help clients with:

  • Accessing in-home support programs.
  • Social connection and reducing loneliness.
  • Connection with home-health in their community.
  • Referrals and access to mental health and addiction supports.
  • Accessing financial benefits and income support.
  • Navigating support for medication costs.
  • Finding help with transportation.

Eligibility: Individuals 55+ living in the City of Calgary, requiring the need for support after hospital discharge.

Referral: Referrals must be through a social worker, hospital, health care professional.
